When I shot film, I  used to have no problem printing 4 x 5 from a medium format.  I accumulated  a bunch of 4 x 5 matts.  I don’t know   any photo lab that will print a 4 x 5 image so my best bet is to print a 4 x 6  and trim the sides. If I resize  the image and print as 4 x 6  will the people look normal/

These instructions are for PhotoShop CS5, but I would be very
surprised if they didn't match what you've got.

Under Image, select Image Size... .
Make sure your image's size is 4 x 6 inches. If it is not, change it
to that size. Click OK, and then select Image / Image Size again.
Near the bottom will be a set of check boxes, one of which is labeled
"Constrain Proportions". Uncheck that box.
Change the size of the long dimension to 5 inches. Click OK.

Your image is now 4 x 5 inches. Everything will look narrower than it
did before. 5/6 as wide, to be exact.

The "Content-Aware Scaling" feature in newer versions of PS tries to
identify important features in the photo and not shrink them as much
as unimportant features, so people won't look as squished.
