6 (saving the merged images as 05B_Working.psd), then adding the
levels adjustment layer, and can't replicate your problem.
A couple things you can try.
Recopy the three images to your hard disk, in case one was somehow
overwritten or corrupted. Unlikely, but who knows?
Delete 05B_Working.psd.
Close and restart PS with the Ctrl-Alt-Shift combo to reset the
defaults again, and give it another shot.
If that doesn't work... I'm not sure what to try.
